ENA's Doctor on the Edge raised the emotional stakes in Episode 6, pushing its central romance into uncertain territory as a long-held secret fractured the trust between its two leads.
The latest episode followed Do Ji-ui, played by Lee Jae-wook, and Yuk Ha-ri, portrayed by Shin Ye-eun, after they finally acknowledged their feelings for one another. Their relationship quickly faced a new challenge, however, when Ha-ri's grandmother, Oh Mi-ja, made a life-altering decision regarding her treatment.
Oh Mi-ja, played by Gil Hae-yeon, ultimately chose to refuse life-sustaining treatment, believing that prolonging her life would only place a greater burden on her granddaughter. After learning her wishes, Do Ji-ui agreed to respect the decision and kept the matter from Ha-ri.
That choice became the emotional centerpiece of the episode. While Do Ji-ui remained determined to protect both women, his silence created a growing divide that neither side could ignore.
The episode also expanded its broader conflicts. An accident tied to a controversial construction project heightened tensions across Pyeondong Island, while administrative changes within the local medical system added further uncertainty. Elsewhere, Eom Jeong-seon and Yong Ju-cheon took a meaningful step forward in their relationship after overcoming a series of misunderstandings.
Do Ji-ui's temporary transfer to Yeopung County provided another source of frustration. Reduced to serving as the public face of a healthcare campaign rather than treating patients directly, he openly criticized the situation during an interview, stating that doctors should remain beside their patients. The remarks ultimately paved the way for his return.
His homecoming, however, offered little relief. By the time he arrived, Ha-ri had already learned the truth about her grandmother's decision and confronted him over his decision to keep it hidden. Unable to explain his actions, Do Ji-ui found himself facing the consequences of the choice he believed was necessary.
The confrontation culminated in the episode's defining moment when Ha-ri asked, "Can't you stop being my doctor and be my boyfriend instead?" Rather than accepting her feelings, Do Ji-ui responded with an apology, leaving their relationship at a standstill and delivering the episode's most painful emotional turn.
With trust now broken and emotions running high, Doctor on the Edge heads into its next chapter with its central relationship facing greater uncertainty than ever before.
